DEPARTMENT OFFICE OR BRING PACKET WITH YOU TO FALL ORIENTATION.
Step 3 [Packet B]: To be completed prior to date of team physical.
BRING THIS PACKET WITH YOU TO YOUR SCHEDULED TEAM PHYSICAL.
**This packet will not be accepted in the athletic department.
